Warning of potent opioid drug found in powder on NSW Central Coast

Health authorities are warning a potent opioid, isotonitazene, has been detected in drugs in a yellow powder form on the NSW Central Coast. The drug may be related to recent deaths which are under investigation.

NSW Health Pathology’s Foren­sic & Ana­lyt­i­cal Sci­ence Ser­vice has col­lab­o­rat­ed with NSW Health, NSW Police and NSW Poi­sons Infor­ma­tion Cen­tre to release the lat­est drug warning.

Med­ical Direc­tor of the NSW Poi­sons Infor­ma­tion Cen­tre, A/Prof Dar­ren Roberts, said drugs con­tain­ing a potent opi­oid such as iso­toni­tazene can cause unex­pect­ed and severe over­dose or death.

Nitazenes can be as strong, or stronger than fen­tanyl and may be more like­ly to affect breath­ing than oth­er opioids.

NSW Health Pathology’s Illic­it Drug Analy­sis and Foren­sic Tox­i­col­o­gy teams work every day to keep the com­mu­ni­ty safe by con­duct­ing expert analy­sis of blood, urine and oral flu­id sam­ples as well as test­ing illic­it drugs seized by police.

Their work sup­ports the health and jus­tice sys­tems and con­tributes to pub­lic health alerts such as this.

What is your position?

Tech­ni­cal Offi­cer at the Grif­fith Lab­o­ra­to­ry. I’ve been with NSW Health Pathol­o­gy for 7 years, and before that, I worked in pri­vate pathol­o­gy in Syd­ney for 3 years.

 

Are you glad you made the move?

Yes, I am. And I’m hap­py here. It’s a love­ly place and the work­ing envi­ron­ment is real­ly friendly.

I had hoped to work with NSW Health Pathol­o­gy.  That was my ambi­tion. I want­ed to learn more skills in a mul­ti­dis­ci­pli­nary core lab setting.

At oth­er labs, you might only get expe­ri­ence in one area, such as chem­istry and be flu­ent in that dis­ci­pline. But here, you can work across mul­ti­ple dis­ci­plines like our blood bank, chem­istry, haema­tol­ogy and blood film mor­phol­o­gy. Pret­ty much everything.

We have a great team here, real­ly supportive.

 

How much of a dif­fer­ence will the new Grif­fith lab­o­ra­to­ry make day to day? 

It will be new, big­ger and a fresh start for us. We’ll also have more space and more instru­ments too which will make a dif­fer­ence. It’s excit­ing to have a brand new lab.

 

How has COVID changed things up a bit these last few years?

It has been busy, but because we worked close­ly as a team it was good. Every­one helped each oth­er out and togeth­er we got the job done. That’s one of the best parts of a small­er, region­al lab­o­ra­to­ry. It’s all hands-on deck when times are tough.

 

What made you want to have a career in pathology?

Pathol­o­gy is all about peo­ple and giv­ing bet­ter patient care. Pathol­o­gy is respon­si­ble for 70% of patient diag­noses. Doc­tors depend on pathol­o­gy. I want­ed to study pathol­o­gy and in fact I am still learn­ing. You can’t stop study­ing. Pathol­o­gy is evolv­ing all the time, with new meth­ods and new sys­tems, it’s con­stant­ly chang­ing, and you’re always learning.

 

What’s your favourite thing about liv­ing in the Grif­fith area?

This is a charm­ing town. When I made the move from Syd­ney, I was­n’t expect­ing it to be this nice, but that’s why I’ve stayed here for 7 years. It has a big Ital­ian com­mu­ni­ty and great restau­rants. My 5‑year-old daugh­ter goes to a good local school. Even though it is far from big cities, as a fam­i­ly, we’ve got every­thing we need right here.

What’s inside that vape?

We hear a lot about the dangers of vaping, but what’s actually inside them? NSW Health Pathology’s Clinical and Environmental Toxicology laboratory has been tasked with finding out.

The fed­er­al gov­ern­ment has announced it will ban recre­ation­al vap­ing and lim­it the sale of vapes to phar­ma­cies, with health experts con­cerned about the rise of vap­ing, par­tic­u­lar­ly among children.

NSW Health evi­dence sug­gests chil­dren are access­ing nico­tine vap­ing prod­ucts in increas­ing num­bers and many adults are also access­ing ille­gal nico­tine vap­ing products.

Since 2015, NSW Health Pathology’s Foren­sic & Ana­lyt­i­cal Sci­ence Service’s Clin­i­cal and Envi­ron­men­tal Tox­i­col­o­gy Lab­o­ra­to­ry has been test­ing e‑cigarettes (or vapes), to deter­mine their con­tents and assist the NSW Min­istry of Health with its sur­veil­lance and pub­lic health inves­ti­ga­tion activities.

Lab­o­ra­to­ry man­ag­er Iri­ni Roume­li­o­tis explains the need for cau­tion when open­ing the devices to test the contents.

“We have to use tools and a fair amount of force to get them open. Some prod­ucts have sparked and caught alight when we’ve tak­en them apart for test­ing,” Ms Roume­li­o­tis said.

“As a result, the team has devel­oped a safe work pro­ce­dure, ensur­ing there is always a sec­ond team mem­ber present dur­ing dis­as­sem­bling of devices.

“Sam­ple prepa­ra­tion is also very labour-inten­sive and involves lots of dilu­tions for each sample.

“Sam­ple extracts are typ­i­cal­ly dilut­ed to between 1 in a mil­lion and 1 in 4 mil­lion to ensure the devices used for analy­sis are not over­loaded with nico­tine, caus­ing con­t­a­m­i­na­tion issues.”

Most of the devices test­ed are found to con­tain nico­tine, even though many con­tain no ref­er­ence to nico­tine on the label.

The typ­i­cal con­cen­tra­tion of nico­tine found in vapes is 5 to 400 mg/cartridge or 20,000- 50,000 mg/L. The aver­age cig­a­rette con­tains about 12 mg of nicotine.

Ms Roume­li­o­tis says in addi­tion to nico­tine, oth­er chem­i­cals and met­als found inside vapes include:
• Flavourants such as diacetyl & Dike­tones flavourings
• Volatile organ­ic com­pounds, VOCs (such as benzene)
• Alde­hy­des (e.g., formalde­hyde, acetalde­hyde, and acrolein)
• Heavy met­als, such as Mer­cury, Lead, Chromi­um, Cad­mi­um, and Arsenic

”There is a per­cep­tion in the com­mu­ni­ty that vap­ing is a safer alter­na­tive to smok­ing tobac­co cig­a­rettes. How­ev­er, intro­duc­ing any chem­i­cals into the human body, espe­cial­ly direct­ly to the lungs as vap­ing does, is not safe,” Ms Roume­li­o­tis said.

“The new­er vape devices are also typ­i­cal­ly get­ting larg­er in size and more potent, con­tain­ing high­er con­cen­tra­tions of nicotine.”

Findings “a wake up call”

Res­pi­ra­to­ry Phys­i­ol­o­gist at Avon­dale Uni­ver­si­ty and expert in the field of nico­tine addic­tion Pro­fes­sor Renee Bit­toun said the results of the lab­o­ra­to­ry test­ing should be a wake-up call.

“The use of these prod­ucts among school-aged and young peo­ple is of great con­cern,” Prof Bit­toun said.

“E‑cigarettes con­tain­ing nico­tine are addic­tive. There is lim­it­ed evi­dence that vapes help peo­ple quit smok­ing but are in fact becom­ing a gate­way to smok­ing tobac­co in adolescents.”

Prof Bit­toun said all vape users are exposed to chem­i­cals and tox­ins that have the poten­tial to cause harm and more research is need­ed to mea­sure how much is being inhaled by young users.

Providing NSW Drug Courts with fast, accurate urine testing

The NSW Health Pathology Drug Toxicology Unit has been crucial to the success of the state’s Drug Court, which recently expanded its operations with the opening of a new Court in Dubbo.

The Drug Tox­i­col­o­gy Unit (DTU) pro­vides inde­pen­dent expert drug test­ing and is recog­nised as the top facil­i­ty of its kind in Australia.

Recent imple­men­ta­tion of new tech­nol­o­gy and urine drug test­ing pro­ce­dures have allowed the DTU to keep pace with the expan­sion of the NSW Drug Court program.

The Drug Court of NSW is a spe­cial­ist court that sits in Par­ra­mat­ta, Toron­to, Syd­ney and now at Dub­bo, which opened in Feb­ru­ary 2023. It is a mul­ti-agency response to drug-relat­ed offend­ing, proven to be more effi­cient at dri­ving down crime and reduc­ing the over­all bur­den on the jus­tice system.

It super­vis­es the inten­sive com­mu­ni­ty-based reha­bil­i­ta­tion of eli­gi­ble drug-depen­dent offend­ers who would oth­er­wise be sen­tenced to full-time imprisonment.

Par­tic­i­pants under­go a 12-month pro­gram, receiv­ing treat­ment for drug depen­den­cy and under­go­ing super­vised urine drug test­ing up to three times a week in the com­mu­ni­ty to ensure they remain drug-free.

A decade-long study found the re-offend­ing rate among Drug Court par­tic­i­pants was 17 per cent low­er than for peo­ple out­side the program.

NSW Health Pathology’s Drug Tox­i­col­o­gy Unit pro­vides essen­tial rapid urine test­ing to ensure Drug Court par­tic­i­pants are abid­ing by the terms of the program.

Retired Judge Roger Dive who head­ed the spe­cial­ist Drug Court for 17 years said the fast turn­around of results from the DTU has been cru­cial to the court’s success.

“Requir­ing super­vised drug tests and hav­ing them reli­ably test­ed is a foun­da­tion of the pro­gram,” he said.

“The Drug Tox­i­col­o­gy Unit has been such an impor­tant part­ner in our work, and the speed with which we receive the results is the envy of Drug Courts around the world.”

“It is just so much more effec­tive to talk about a test tak­en only two or three days ago, as apart from try­ing to work out what hap­pened weeks ago.”

DTU lab man­ag­er Shan­mugam Banuku­mar (pic­tured above) says the lab has under­gone a tech­no­log­i­cal trans­for­ma­tion in recent years, ensur­ing it can man­age the increased demand from the Drug Court, con­tin­u­al­ly pro­vid­ing fast, accu­rate results.

“Pre­vi­ous­ly there was a lot of man­u­al han­dling of sam­ples, but we’ve now installed world-lead­ing auto­mat­ed machin­ery that has com­plete­ly replaced all the man­u­al extrac­tion processes.

“All the sam­ples are tracked through­out the process, ensur­ing the integri­ty of sam­ple processing.”

The instru­ments used by the DTU to detect illic­it drugs have all been upgrad­ed and replaced with the lat­est tech­nol­o­gy, four liq­uid chro­matog­ra­phy mass spec­trom­e­ters, capa­ble of test­ing for up to 80 dif­fer­ent drugs in a mat­ter of minutes.

A col­lab­o­ra­tion between sci­ence and jus­tice, help­ing to reduce the social and eco­nom­ic harms of drug mis­use with­in the NSW community.

New Point of Care Testing device a win for rural health

NSW’s most remote hospitals will soon perform a key, potentially life-saving blood test at the patient’s bedside and receive results within minutes, providing clinicians with fast, accurate results to inform patient care.

NSW Health Pathol­o­gy is part­ner­ing with five region­al Local Health Dis­tricts to intro­duce a new Point of Care Test­ing device, the Pix­Cell Hemo­screen, in 30 hos­pi­tals across the state with­out on-site pathol­o­gy labs.

The device will allow doc­tors and nurs­es to per­form one of the most rou­tine­ly ordered pathol­o­gy tests, a Full Blood Count, to detect poten­tial­ly life-threat­en­ing con­di­tions such as sep­sis, anaemia, bleed­ing and blood clot­ting prob­lems (ie low platelet counts).

The devices will be rolled-out to ser­vices in com­ing months, along with train­ing to sup­port hos­pi­tal staff to safe­ly and effec­tive­ly oper­ate the machine and to fol­low-up on abnor­mal results.

Gayle Warnock, Act­ing Oper­a­tions Man­ag­er, NSW Health Pathology’s Point of Care Test­ing Ser­vice, said the device will pro­vide blood test results with­in six min­utes, rather than wait­ing for blood sam­ples to be trans­port­ed and test­ed at a lab some­times hours away.

“This is a game chang­er for hos­pi­tals that do not have on-site pathol­o­gy labs and the patients they care for,” Galye said.

“Tra­di­tion­al­ly, these rur­al and remote hos­pi­tals would take a blood sam­ple and have it couri­ered to the clos­est NSW Health Pathol­o­gy lab for analy­sis, some­times hun­dreds of kilo­me­tres away.

“Now clin­i­cians can per­form this test instant­ly at the patient’s bed­side so they can make time­ly, informed deci­sions about patient care. This could include whether the per­son can con­tin­ue receiv­ing care in their local hos­pi­tal or if they need to be trans­ferred to a more spe­cialised facil­i­ty for treat­ment,” Gayle said.

“Repeat­ing the Full Blood Count test dur­ing the patient’s treat­ment also helps hos­pi­tal staff under­stand whether the person’s con­di­tion is improving.”

Our Point of Care Test­ing ser­vice com­plet­ed an exten­sive tri­al peri­od, known as ver­i­fi­ca­tion, of the Pix­Cell device in con­sul­ta­tion with our expe­ri­enced haema­tol­o­gists pri­or to start­ing the roll-out.

A clear process was put into place to ensure all sam­ples that are abnor­mal, or from patients with known blood dis­or­ders, are reviewed by a haematologist.

“These sam­ples will be repeat­ed in lab­o­ra­to­ries via our usu­al test­ing process­es to ensure patients, and the hos­pi­tal staff car­ing for them, receive high-qual­i­ty, accu­rate pathol­o­gy results to inform their care,” Gayle said.

“NSW Health Pathol­o­gy will also close­ly work with hos­pi­tal staff to mon­i­tor the over­all ser­vice and per­for­mance of the devices, and ensure they’re man­aged to the same high stan­dards as tra­di­tion­al lab instruments.”

Drug alert: High dose MDMA tablets in circulation

NSW Health is warning the community about high dose MDMA tablets which have been found to contain twice the dose of MDMA usually found in so-called “ecstasy” tablets.

 

The tablets are orange in colour, spade-shaped, and often marked with an ‘Ace of Spades’ logo.

 

There is an increased risk of harm when tak­ing high­er-dose or mul­ti­ple MDMA tablets. MDMA is often poor­ly man­u­fac­tured and the amount in a tablet can vary con­sid­er­ably even in the same batch.

 

High dose MDMA use has been linked to death and oth­er seri­ous harms that result in inten­sive care hos­pi­tal admissions.
